Georgia Conditional Employee or Food Employee Reporting Agreement
This purposes of this agreement document from the GA Department of Public Health is to prevent transmission of diseases through food by infected employees. Conditional employees or food employees have a responsibility to notify the person in charge when they experience any of the conditions listed so that the person in charge can take appropriate steps to preclude the transmission of foodborne illness.
