NEW FOOD SERVICES:
What do I need to do prior to opening a Food Service? (Including but not limited to the following):
Submit at least fourteen (14) days prior to beginning construction/remodeling:
- Application for Food Service Establishment
- Permit and Plan Review Fees (Fee Schedule)
- Copy of Proposed Menu
- Set of plans for new establishments or kitchen diagram for existing establishments

- Verification of Residency Form (In accordance with the Georgia Department of Public Health, Alien Residency Verification Policy (Policy# GC-090008))
- **Please note, all individuals or corporations who obtain a permit (i.e. food service, swimming pools, body art) from DPH requires completion of secure and verifiable information to verify citizenship/immigration status.
