What do I need to do prior to opening a Food Service? (Including but not limited to the following):

Submit at least fourteen (14) days prior to beginning construction/remodeling:

  • Food Service Application
  • and Plan Review Fees (Fee Schedule)
  • Copy of Proposed Menu
  • Set of plans for new establishments or kitchen diagram for existing establishments

  • Verification of Residency – revised April 2014 (In accordance with the Georgia Department of Public Health, Alien Residency Verification Policy (Policy# GC-090008))
    • **Please note, all individuals or corporations who obtain a permit (i.e. food service, swimming pools, body art) from DPH requires completion of secure and verifiable information to verify citizenship/immigration status.
      In accordance with O.C.G.A. Section 50-36-1(e)(2) Secure and Verifiable Documents Under O.C.G.A. § 50-36-2
